PUBLIC HEALTH SERVICES:
A professional
Registered Nurse provides public health nursing services in the areas of health
promotion, health assessment and surveillance, injury and disease prevention to
individuals, families and groups throughout their life span.
Services may include:
- Flu Clinics
- Immunization Clinics
- Prenatal Support
- Breastfeeding Support
- Postpartum Support
- Parenting information
- Travel Health information and
immunizations
- Sexual Health information and education
- Dental Services for children
- Nutrition information
- Car and Booster Seat tips and training
- Promotion of Health Living
COMMUNITY CARE
The goal of Community Care is to assist clients to achieve and maintain
health, well-being, and independence in their homes.
Community Care services are open to clients of all ages,
who require health services in order to remain in their home. Clients may access the program for Short
Term services, Long Term services or Palliative Care services. A number of different professionals work in
Community Care. Services offered
include:
NURSING CARE
Nursing Care is provided by professional Registered
Nurses and Licensed Practical Nurses.
Services may include:
- assessment of client need
and coordination of services
- health monitoring and
teaching of care needs
- post-surgery care such as
dressing changes, antibiotics,
- pain control, Home IV
Program, care of PICC lines, central line
- monitoring of special
care treatment such as peritoneal dialysis
- hospital discharge
planning
REHABILITATION SERVICES
Occupational,
respiratory and physical therapy provide assessment, treatment, evaluation and
education to individuals of all ages.
Services may include:
- assessment for adaptive
equipment through Alberta Aids to Daily Living (AADL) e.g bathing aides,
wheelchairs,
- Walkers **Clients may
be required to cost share
- cognitive, perceptual,
fine and gross motor assessment
- home safety assessment
- activities of daily
living assessment
- home modification
assessment
SUPPORT SERVICES
Support services are provided by trained Health Care
Aides.
Services may include:
Bathing, dressing, grooming assistance
Morning and evening care
Bowel routines, colostomy care
Respite service (relief break for a primary
caregiver)
Assistance with exercise program
Medication Assistance Program
CONSULTATIVE SERVICES
When required,
Community Care staff consult with a Social Workers, Dietitians, Pharmacists,
Respiratory Therapists and other health care professionals
COST
There is no
cost for Community Care Services
